Insurance

PLEASE NOTE: I am currently concentrating on an increased training schedule and have reduced my weekly office hours. I am therefore unable to accept new clients for the time being. Feel free to check back for updates.

My counseling services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions, and be alert of policy changes as they occur:

* Does my health insurance plan include mental/behavioral health benefits?
* What is my deductible and has it been met?
* How many sessions per year does my insurance cover?
* What is the amount per session? What is my co-pay (co-insurance)?
* Is approval required from my primary care physician?
* If I have a Medical Savings Account, how do I submit for reimbursement?

INSURANCE COMPANIES ACCEPTED:
I am currently accepting clients from the following companies as my schedule will allow. My bookkeeper will bill and track insurance claims for these companies.

OTHER INSURANCE:

In addition, the following insurance companies, and many others not listed, may pay for my services "out-of-network". In these cases, your full payment is required at each session, though my bookkeeper will submit claims asking insurance to reimburse you directly.

Ask your company all of the questions above as well as:
Do I have an "Out-of-Network" benefit?
Do you cover services by Marriage & Family Therapists?

EAP/EMPLOYEE ASSISTANCE CONTRACTS:
Your employer, or a family member's employer, may allow you to see me for a limited number of sessions for assessment, counseling, and referral at no cost to you under an Employee Assistance Plan benefit. This benefit is separate from a health insurance policy though may be an extra service from that insurance company or a separate EAP service. Please check your employee manual or with the employer's Human Resources department for more information.
